As of the date above, I am submitting my 30 day notice of cancellation.

I am responsible for any past due fees, and any fees accrued during this 30 day notice. Fees include monthly dues, swim meet fees and any other outstanding/accrued invoices.

If these fees are not paid, your account will not be suspended and will accrue monthly fees until these invoices/fees are paid in full.
